Von Eifersucht gequält, scheint Jazzmusiker Fred Madison seine Frau Renee blutig abgeschlachtet zu haben. Ohne sich an die Tat erinnern zu können, landet er abgeurteilt im Gefängnis. Dort verschwindet er spurlos, und ein junger Automechaniker mit Gedächtnisverlust lebt seinen Alptraum fort, in dem die ermordete Renee wiederaufzuerstehen scheint.

Jazz saxophonist Fred Madison's (Bill Pullman) marriage is on the rocks. When he is arrested after a party and accused of murdering his wife Renee (Patricia Arquette), he ends up on death row. Plagued by severe headaches and visions of the Mystery Man (Robert Blake), Fred transforms into car mechanic Pete Dayton (Balthazar Getty). The guards who find the innocent stranger in the cell have to let Pete go. But it soon turns out that everyone, trapped in their own fate, is lost... on the Lost Highway.
